Variant summary: HPS1 c.716T>C (p.Leu239Pro) results in a non-conservative amino acid change located in the FUZ/MON1/HPS1, second Longin domain (IPR043971) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The frequency data for this variant in gnomAD is considered unreliable, as metrics indicate poor data quality at this position. c.716T>C has been reported in the literature in multiple individuals affected with Hermansky-Pudlak Syndrome or Inherited albinism (example, Lasseaux_2017, Hermos_2002). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 31589614, 12442288, 29345414). ClinVar contains an entry for this variant (Variation ID: 1452567). Based on the evidence outlined above, the variant was classified as pathogenic. -

The p.Leu239Pro variant in HPS1 has been reported in 3 individuals with Hermansky-Pudlak syndrome (PMID: 12442288, 29345414), and has been identified in 0.003% (30/1149276) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs281865080). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (VCV001452567.7) and has been interpreted as likely pathogenic by Women's Health and Genetics/Laboratory Corporation of America, Baylor Genetics, and Labcorp (Formerly Invitae). Of the 3 affected individuals, at least 2 were compound heterozygotes that carried a likely pathogenic/pathogenic variant in trans (PMID: 12442288, 29345414), which increases the likelihood that the p.Leu239Pro variant is pathogenic. Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive Hermansky-Pudlak syndrome. This sequence change replaces leucine, which is neutral and non-polar, with proline, which is neutral and non-polar, at codon 239 of the HPS1 protein (p.Leu239Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Hermansky-Pudlak syndrome and/or ocular albinism (PMID: 12442288, 29345414). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. This variant is also known as c.922T>C. ClinVar contains an entry for this variant (Variation ID: 1452567). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt HPS1 protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic. -
