Variant summary
Our verdict is Likely benign. The variant received -5 ACMG points: 0P and 5B. BP4_StrongBS2_Supporting
The NM_006267.5(RANBP2):c.8411G>T(p.Ser2804Ile) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000093 in 1,613,618 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 16/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★).

The c.8411G>T (p.S2804I) alteration is located in exon 24 (coding exon 24) of the RANBP2 gene. This alteration results from a G to T substitution at nucleotide position 8411, causing the serine (S) at amino acid position 2804 to be replaced by an isoleucine (I). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
Familial acute necrotizing encephalopathy Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with RANBP2-related disease. This variant is present in population databases (rs368049213, ExAC 0.03%). This sequence change replaces serine with isoleucine at codon 2804 of the RANBP2 protein (p.Ser2804Ile). The serine residue is highly conserved and there is a large physicochemical difference between serine and isoleucine. -
