This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 137 of the NBAS protein (p.Arg137Trp). This variant is present in population databases (rs368085185, gnomAD 0.004%). This missense change has been observed in individuals with clinical features of NBAS-related conditions (PMID: 26286438, 26578240, 28410752, 34906502). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 218416). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on NBAS protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Not observed at significant frequency in large population cohorts (gnomAD); This variant is associated with the following publications: (PMID: 32146038, 26286438, 28031453, 26578240, 28410752, 33042920, 31964843, 38244286, 34906502) -
Acute infantile liver failure due to synthesis defect of mtDNA-encoded proteins Pathogenic:1
Variant summary: NBAS c.409C>T (p.Arg137Trp) results in a non-conservative amino acid change located in the Neuroblastoma-amplified sequence, N-terminal domain (IPR029145) of the encoded protein sequence. Two of four in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251480 control chromosomes (gnomAD). c.409C>T has been reported in the literature in multiple compound heterozygous and homozygous individuals affected with Liver Failure Acute Infantile (Segarra_2015, Staufner_2015, Cardenas_2017, Capo-Chichi_2015). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 26578240, 28410752, 31964843, 28576691, 26286438, 26541327). Two submitters have cited clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as pathogenic and likely pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
