The NOTCH3 c.1729A>G; p.Thr577Ala variant (rs368181126; ClinVar ID: 447797) is reported in the literature in an individual affected with CADASIL, although supporting evidence of pathogenicity was not provided (Ferreira 2007). This variant is found in the general population with an overall allele frequency of 0.002% (7/282,084 alleles) in the Genome Aggregation Database (v2.1.1). Computational analyses are uncertain whether this variant is neutral or deleterious (REVEL: 0.481). Most pathogenic NOTCH3 variants occur in exons 2-24 and either create or destroy a cysteine residue within an EGF-like domain (Rutten 2014). However, there are several amino acid substitutions not involving cysteine that may be disease-associated (Muino 2017). Although the p.Thr577Ala variant does not involve a cysteine residue, due to its low population frequency its clinical significance is uncertain. References: Ferreira S et al.
