The c.2021A>G (p.E674G) alteration is located in exon 6 (coding exon 6) of the RSPH4A gene. This alteration results from a A to G substitution at nucleotide position 2021, causing the glutamic acid (E) at amino acid position 674 to be replaced by a glycine (G). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
This sequence change replaces glutamic acid, which is acidic and polar, with glycine, which is neutral and non-polar, at codon 674 of the RSPH4A protein (p.Glu674Gly). This variant is present in population databases (rs368369580, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with RSPH4A-related conditions. ClinVar contains an entry for this variant (Variation ID: 238208). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
