This missense variant replaces arginine with leucine at codon 827 of the ATP7B protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). This variant alters a conserved arginine residue in the ATP nucleotide-binding of the ATP7B protein (a.a. 786 - 917), a highly conserved region that is considered to be important for ATP7B protein function (PMID: 35245129; ClinVar). To our knowledge, functional studies have not been reported for this variant. This variant has been observed in an individual affected with autosomal recessive Wilson disease, who carried a second pathogenic variant in the same gene (PMID: 24517292). This variant has been identified in 1/249562 chromosomes in the general population by the Genome Aggregation Database (gnomAD). Although there is a suspicion that this variant may be associated with disease, the available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Variant summary: ATP7B c.2480G>T (p.Arg827Leu) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 249562 control chromosomes. c.2480G>T has been reported in the literature in at least two individuals affected with Wilson Disease in the compond heterozygous state (Ferenci_2014, Ferenci_2019). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. ClinVar contains an entry for this variant (Variation ID: 551685). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
