Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 0P and 4B. BP4_ModerateBP6BS2_Supporting
The NM_001042492.3(NF1):c.2573C>G(p.Ser858Cys) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000143 in 1,611,856 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 13/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. S858P) has been classified as Uncertain significance.

Neurofibromatosis, type 1 Uncertain:1Benign:1
not provided Uncertain:1Benign:1
Identified in an individual with neurofibromatosis type I, co-occurring with an NF1 truncating variant (Koczkowska 2018); In silico analysis supports that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 29290338)
Neurofibromatosis, type 1;C0349639:Juvenile myelomonocytic leukemia;C0553586:Café-au-lait macules with pulmonary stenosis;C1834235:Neurofibromatosis, familial spinal;C2931482:Neurofibromatosis-Noonan syndrome Uncertain:1
NF1 NM_000267.3 exon 21 p.Ser858Cys (c.2573C>G): This variant has not been reported in the literature and is present in 0.005% (6/113248) of European alleles in the Genome Aggregation Database (http://gnomad.broadinstitute.org/variant/17-29556206-C-G). This variant is present in ClinVar (Variation ID:185364). Evolutionary conservation and computational predictive tools suggest that this variant may not impact the protein. In summary, data on this variant is insufficient for disease classification. Therefore, the clinical significance of this variant is uncertain.
Hereditary cancer-predisposing syndrome Uncertain:1
The p.S858C variant (also known as c.2573C>G), located in coding exon 21 of the NF1 gene, results from a C to G substitution at nucleotide position 2573. The serine at codon 858 is replaced by cysteine, an amino acid with dissimilar properties. This variant was previously reported in the SNPDatabase as rs369493270. Based on data from the NHLBI Exome Sequencing Project (ESP), the G-allele has an overall frequency of approximately 0.01% (1/13002) total alleles studied,<span style="background-color:initial">having been observed in 0/4406 African American allelesand 0.01% (1/8596) European American alleles. This variant was not reported in the 1000 Genomes Project population-based cohort.<span style="background-color:initial">To date, this alteration has been detected with an allele frequency of approximately 0.004% (greater than 110000 alleles tested) in our clinical cohort.<span style="background-color:initial">This amino acid position is not wellconserved in availablevertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis.<span style="background-color:initial">Since supporting evidence is limited at this time, the clinical significance of<span style="background-color:initial">p.S858C<span style="background-color:initial">remains unclear.

Hereditary cancer-predisposing syndrome;CN230736:Cardiovascular phenotype Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
