Variant summary: PIGN c.776T>C (p.Phe259Ser) results in a non-conservative amino acid change located in the N-terminal domain (IPR037671) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 248578 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.776T>C in individuals affected with Multiple Congenital Anomalies-Hypotonia Syndrome and no experimental evidence demonstrating its impact on protein function have been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and both classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces phenylalanine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 259 of the PIGN protein (p.Phe259Ser). This variant is present in population databases (rs370310838, gnomAD 0.01%). This missense change has been observed in individual(s) with clinical features of PIGN-related conditions (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 451789). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
