This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -
This sequence change replaces methionine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 875 of the DSG2 protein (p.Met875Val). This variant is present in population databases (rs370316475, gnomAD 0.008%). This missense change has been observed in individual(s) with arrhythmogenic right ventricular cardiomyopathy and/or clinical features of DSG2-related conditions (PMID: 21606396, 29517769, 30847666). This variant has been identified in 9/279964 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.M875V variant (also known as c.2623A>G), located in coding exon 15 of the DSG2 gene, results from an A to G substitution at nucleotide position 2623. The methionine at codon 875 is replaced by valine, an amino acid with highly similar properties. This variant co-occurred with a PKP2 nonsense mutation in an individual from an arrhythmogenic right ventricular cardiomyopathy cohort (Cox MG et al. Circulation, 2011 Jun;123:2690-700). This variant has also been detected in a dilated cardiomyopathy (DCM) cohort and a cohort referred for DCM genetic testing; however, clinical details were limited and other genetic findings were present (Herkert JC et al. Genet Med, 2018 11;20:1374-1386; (van Lint FHM et al. Neth Heart J, 2019 Jun;27:304-309). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
