A variant of uncertain significance has been identified in the FGD4 gene. The P17R variant has notbeen published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The P17R variant is observed in 29/126,648 (0.02%) alleles from individuals of European background in large population cohorts (Lek et al., 2016). The P17R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In-silico analyses, including protein predictors and evolutionary conservation, support that this variant does not alter protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rarebenign variant.
Charcot-Marie-Tooth disease type 4H Uncertain:1
This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease.

The p.P17R variant (also known as c.50C>G), located in coding exon 1 of the FGD4 gene, results from a C to G substitution at nucleotide position 50. The proline at codon 17 is replaced by arginine, an amino acid with dissimilar properties. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
Charcot-Marie-Tooth disease type 4 Uncertain:1
This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 17 of the FGD4 protein (p.Pro17Arg). This variant is present in population databases (rs371407163, gnomAD 0.02%). This missense change has been observed in individual(s) with Charcot-Marie-Tooth disease (PMID: 32376792). ClinVar contains an entry for this variant (Variation ID: 246146). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
