The p.Y626F variant (also known as c.1877A>T), located in coding exon 6 of the BLM gene, results from an A to T substitution at nucleotide position 1877. The tyrosine at codon 626 is replaced by phenylalanine, an amino acid with highly similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: BLM c.1877A>T (p.Tyr626Phe) results in a conservative amino acid change located in the BDHCT-box associated domain (IPR032439) of the encoded protein sequence. Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1e-05 in 195556 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1877A>T has been reported in the literature in a cohort of healthy individuals with no personal or family history indicative of a highly penetrant cancer-predisposing germline mutation (Bodian_2014). To our knowledge, no occurrences of the variant in individuals affected with Bloom Syndrome and no experimental evidence demonstrating an impact on protein function have been reported. The following publication has been ascertained in the context of this evaluation (PMID: 24728327). Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and all classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
