Genes affected
ACADM (HGNC:89): (acyl-CoA dehydrogenase medium chain) This gene encodes the medium-chain specific (C4 to C12 straight chain) acyl-Coenzyme A dehydrogenase. The homotetramer enzyme catalyzes the initial step of the mitochondrial fatty acid beta-oxidation pathway. Defects in this gene cause medium-chain acyl-CoA dehydrogenase deficiency, a disease characterized by hepatic dysfunction, fasting hypoglycemia, and encephalopathy, which can result in infantile death. Alternatively spliced transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Jul 2008]

Likely pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpJan 02, 2024This sequence change falls in intron 7 of the ACADM gene. It does not directly change the encoded amino acid sequence of the ACADM protein. It affects a nucleotide within the consensus splice site. This variant is present in population databases (rs375921211, gnomAD 0.02%). This variant has been observed in individual(s) with MCAD deficiency (PMID: 20434380, 30626930). ClinVar contains an entry for this variant (Variation ID: 438863). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant is not likely to affect RNA splicing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

Uncertain significance, criteria provided, single submitterclinical testingGeneDxNov 25, 2019In-silico analysis, which includes splice predictors and evolutionary conservation, is inconclusive as to whether the variant alters gene splicing. In the absence of RNA/functional studies, the actual effect of this sequence change is unknown.; This variant is associated with the following publications: (PMID: 20434380, 30626930) -
ACADM-related disorder Uncertain:1
Uncertain significance, no assertion criteria providedclinical testingPreventionGenetics, part of Exact SciencesJul 11, 2024The ACADM c.599+3A>G variant is predicted to interfere with splicing. This variant has been reported in two individuals with biochemical markers consistent with medium-chain acyl-CoA dehydrogenase deficiency. In both cases, the variant was found in addition to a known ACADM pathogenic variant (c.985A>G and c.1102_1105del) (Navarrete et al 2019. PubMed ID: 30626930; Smith et al. 2010. PubMed ID: 20434380). It has been reported in the compound heterozygous state in an individual with newborn screening consistent with medium chain acyl-CoA dehydrogenase deficiency (Internal Data, PreventionGenetics). This variant is reported in 0.019% of alleles in individuals of European (non-Finnish) descent in gnomAD. Although we suspect that this variant may be pathogenic, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
