Genes affected
SMAD4 (HGNC:6770): (SMAD family member 4) This gene encodes a member of the Smad family of signal transduction proteins. Smad proteins are phosphorylated and activated by transmembrane serine-threonine receptor kinases in response to transforming growth factor (TGF)-beta signaling. The product of this gene forms homomeric complexes and heteromeric complexes with other activated Smad proteins, which then accumulate in the nucleus and regulate the transcription of target genes. This protein binds to DNA and recognizes an 8-bp palindromic sequence (GTCTAGAC) called the Smad-binding element (SBE). The protein acts as a tumor suppressor and inhibits epithelial cell proliferation. It may also have an inhibitory effect on tumors by reducing angiogenesis and increasing blood vessel hyperpermeability. The encoded protein is a crucial component of the bone morphogenetic protein signaling pathway. The Smad proteins are subject to complex regulation by post-translational modifications. Mutations or deletions in this gene have been shown to result in pancreatic cancer, juvenile polyposis syndrome, and hereditary hemorrhagic telangiectasia syndrome. [provided by RefSeq, May 2022]

Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsOct 25, 2024The c.1139G>A pathogenic mutation (also known as p.R380K), located in coding exon 8 of the SMAD4 gene, results from a G to A substitution at nucleotide position 1139. The amino acid change results in arginine to lysine at codon 380, an amino acid with highly similar properties. However, this change occurs in the last base pair of coding exon 8, which makes it likely to have some effect on normal mRNA splicing. This variant has been observed in multiple individuals with a personal and/or family history that is consistent with juvenile polyposis (Aretz S et al. J Med Genet, 2007 Nov;44:702-9; Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Aretz S et al. J Med Genet, 2007 Nov;44:702-9; Ambry internal data). In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the supporting evidence, this variant is interpreted as a disease-causing mutation. -
Juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome Pathogenic:1
Likely pathogenic, criteria provided, single submitterclinical testingMyriad Genetics, Inc.Feb 09, 2024This variant is considered likely pathogenic. This variant has been reported in multiple individuals with clinical features of gene-specific disease [PMID: 17873119, 25931195]. mRNA analysis has demonstrated abnormal mRNA splicing occurs [PMID: 17873119]. -
Juvenile polyposis syndrome Pathogenic:1
Likely pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpJul 05, 2019In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Nucleotide substitutions within the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Experimental studies have shown that this missense change led to the formation of a cryptic splice site within exon 8 that is expected to result in a premature stop codon due to a frameshift (PMID: 17873119). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has been observed in individuals affected with juvenile polyposis syndrome (PMID: 17873119, Invitae). ClinVar contains an entry for this variant (Variation ID: 38283). This variant is not present in population databases (ExAC no frequency). This sequence change replaces arginine with lysine at codon 380 of the SMAD4 protein (p.Arg380Lys). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and lysine. This variant also falls at the last nucleotide of exon 9 of the SMAD4 coding sequence, which is part of the consensus splice site for this exon. -
