This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). Publications were found based on this search. However, the evidence from the literature, in combination with allele frequency data from public databases where available, was not sufficient to rule this variant in or out of causing disease. Therefore, this variant is classified as a variant of unknown significance.
This sequence change replaces cysteine, which is neutral and slightly polar, with tyrosine, which is neutral and polar, at codon 96 of the PPT1 protein (p.Cys96Tyr). This variant is present in population databases (rs386833640, gnomAD 0.004%). This missense change has been observed in individual(s) with neuronal ceroid lipofuscinosis (PMID: 23772246; internal data). ClinVar contains an entry for this variant (Variation ID: 56188). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt PPT1 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.

The c.287G>A (p.C96Y) alteration is located in exon 3 (coding exon 3) of the PPT1 gene. This alteration results from a G to A substitution at nucleotide position 287, causing the cysteine (C) at amino acid position 96 to be replaced by a tyrosine (Y). Based on data from gnomAD, the A allele has an overall frequency of 0.001% (4/282852) total alleles studied. The highest observed frequency was 0.003% (4/129170) of European (non-Finnish) alleles. This variant has been identified in conjunction with another PPT1 variant in an individual with features consistent with PPT1-related neuronal ceroid lipofuscinosis (Khan, 2013). This amino acid position is highly conserved in available vertebrate species. This alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, this alteration is classified as likely pathogenic.

Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 1.2e-05 in 251464 control chromosomes. c.287G>A has been observed in compound heterozygous individuals affected with Neuronal Ceroid-Lipofuscinosis (Batten Disease) (Mole_2001, Khan_2013, Setty_2013, LCG internal data). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 23772246, 11589012, 24082928). ClinVar contains an entry for this variant (Variation ID: 56188). Based on the evidence outlined above, the variant was classified as likely pathogenic.
