Variant summary

Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong

The NM_000038.6(APC):c.1500T>A(p.Tyr500Ter) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. Y500Y) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Genes affected
APC (HGNC:583): (APC regulator of WNT signaling pathway) This gene encodes a tumor suppressor protein that acts as an antagonist of the Wnt signaling pathway. It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. Familial adenomatous polyposis 1 Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testingMyriad Genetics, Inc.May 01, 2023This variant is considered pathogenic. This variant creates a termination codon and is predicted to result in premature protein truncation. -
Pathogenic, criteria provided, single submitterclinical testingInvitaeDec 31, 2019For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in APC are known to be pathogenic (PMID: 17963004, 20685668). This variant has been observed in individuals with clinical features of familial adenomatous polyposis (PMID: 19725996). ClinVar contains an entry for this variant (Variation ID: 217929). This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Tyr500*) in the APC gene. It is expected to result in an absent or disrupted protein product. -
Hereditary cancer-predisposing syndrome Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testingColor Diagnostics, LLC DBA Color HealthNov 27, 2019This variant changes 1 nucleotide in exon 12 of the APC gene, creating a premature translation stop signal. This variant is expected to result in an absent or non-functional protein product. Splice site prediction tools suggest that this variant may not impact RNA splicing. To our knowledge, functional studies have not been performed for this variant. This variant has been reported in several individuals affected with Familial Adenomatous Polyposis (FAP) (PMID: 19029688, 19725996, 20007843). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of APC function is a known mechanism of disease. Based on the available evidence, this variant is classified as Pathogenic. -
Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsNov 29, 2023The p.Y500* pathogenic mutation (also known as c.1500T>A), located in coding exon 11 of the APC gene, results from a T to A substitution at nucleotide position 1500. This changes the amino acid from a tyrosine to a stop codon within coding exon 11. This alteration has been detected in multiple individuals with a clinical diagnosis of Familial Adenomatous Polyposis (FAP) (Pawski A et al. Hered Cancer Clin Pract, 2007 Dec;5:195-8). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
