This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 105 of the NCF4 protein (p.Arg105Gln). This variant is present in population databases (rs387906808, gnomAD 0.008%). This missense change has been observed in individual(s) with chronic granulomatous disease and/or symptoms of chronic granulomatous disease (PMID: 19692703, 29969437). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 30194). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects NCF4 function (PMID: 19692703, 29969437). For these reasons, this variant has been classified as Pathogenic. -

Variant summary: NCF4 c.314G>A (p.Arg105Gln) results in a conservative amino acid change located in the Phox homology domain (IPR001683) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.8e-05 in 251406 control chromosomes. c.314G>A has been reported in the literature in both compound heterozygous and homozygous individuals affected with Chronic Granulomatous Disease (e.g., Matute_2009, vandeGeer_2018), and the variant was shown to segregate with disease in related individuals. These data indicate that the variant is very likely to be associated with disease. At least two publications report experimental evidence evaluating an impact on protein function, finding that the variant results in greatly reduced intracellular NADPH oxidase activity in vitro (e.g., Matute_2009, vandeGeer_2018), although the effect of the variant on NADPH oxidase activity in homozygous patient cells was variable depending on the stimulus (e.g., vandeGeer_2018). The following publications have been ascertained in the context of this evaluation (PMID: 19692703, 29969437). One submitter has reported clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
