This duplication of one nucleotide in BRCA2 is denoted c.4470dupA at the cDNA level and p.Leu1491ThrfsX23 (L1491TfsX23) at the protein level. The normal sequence, with the bases that are duplicated in brackets, is AAAT[A]CTGA. The duplication causes a frameshift, which changes a Leucine to a Threonine at codon 1491, and creates a premature stop codon at position 23 of the new reading frame. Although this variant has not, to our knowledge, been reported in the literature, it is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. we consider this variant to be pathogenic. -

Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 250468 control chromosomes. c.4470dupA has been reported in the literature in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (e.g., Kim_2017) as well as individuals affected with prostate cancer (e.g., Robinson_2017, Wu_2018). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014, and all laboratories classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
