This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
This missense variant replaces alanine with threonine at codon 262 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function. A functional study has reported that this variant may have a partial loss-of-function with mild sensitivity to PARP inhibitor and mislocalization to sites of DNA damage, but no sensitivity to cisplatin, MMC and other DNA damaging agents in Brca2-deficient mouse embryonic stem cells (PMID: 32393398). This variant has been reported in an individual affected with breast and ovarian cancer (PMID: 32393398), and it has been detected in a breast cancer case-control meta-analysis in 1/60466 cases and 1/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID BRCA2_007756). A multifactorial analysis has reported a likelihood ratio based on personal and family history of 0.538 from log(LR)=-0.269334614 (PMID: 31853058). This variant has been identified in 2/249784 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Three of four in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 249784 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.784G>A has been reported in the literature in at least one individual affected with Hereditary Breast And Ovarian Cancer Syndrome (e.g. Sirisena_2020). This report does not provide unequivocal conclusions about association of the variant with disease. At least one publication has reported experimental evidence evaluating the variant and found that it had a very mild effect on protein function (e.g. Sirisena_2020). Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and both classified the variant as uncertain significance. In the published literature, the variant has been reported in an affected individual with breast and ovarian cancer with a family history of cancer (PMID: 32393398 (2020)), as well as in a breast cancer case and a control individual in a large scale breast cancer association study (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/BRCA2)). A functional study observed a reduction in homologous recombination in a green fluorescent protein (GFP)-based reporter assay as compared to wild type (PMID: 32393398 (2020)). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%.
