Variant summary

Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong

The NM_003079.5(SMARCE1):c.715C>T(p.Arg239Ter) variant causes a stop gained, splice region change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 3/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Pathogenic (★★). Variant results in nonsense mediated mRNA decay.

Genes affected
SMARCE1 (HGNC:11109): (SWI/SNF related, matrix associated, actin dependent regulator of chromatin, subfamily e, member 1) The protein encoded by this gene is part of the large ATP-dependent chromatin remodeling complex SWI/SNF, which is required for transcriptional activation of genes normally repressed by chromatin. The encoded protein, either alone or when in the SWI/SNF complex, can bind to 4-way junction DNA, which is thought to mimic the topology of DNA as it enters or exits the nucleosome. The protein contains a DNA-binding HMG domain, but disruption of this domain does not abolish the DNA-binding or nucleosome-displacement activities of the SWI/SNF complex. Unlike most of the SWI/SNF complex proteins, this protein has no yeast counterpart. [provided by RefSeq, Jul 2008]

Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpOct 21, 2022For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 55832). This premature translational stop signal has been observed in individual(s) with spinal meningiomas (PMID: 23377182). This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Arg239*) in the SMARCE1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in SMARCE1 are known to be pathogenic (PMID: 23377182). -

Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsNov 30, 2022The p.R239* variant (also known as c.715C>T), located in coding exon 8 of the SMARCE1 gene, results from a C to T substitution at nucleotide position 715. This changes the amino acid from an arginine to a stop codon within coding exon 8. This alteration has been reported in affected relatives from a family with spinal meningiomas (Smith MJ et al. Nat Genet, 2013 Mar;45:295-8; Smith MJ et al. Histopathology, 2017 Apr;70:814-820). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Loss-of-function variants in SMARCE1 are known to cause increased risk of meningiomas; however, such associations with neurodevelopmental disorders are exceedingly rare (Kosho T et al. Am J Med Genet C Semin Med Genet. 2014 Sep;166C(3):262-75; Smith JM et al. Nat Genet. 2013 Mar;45(3):295-8). Based on the supporting evidence, this alteration is pathogenic for an increased risk of meningiomas; however, the association of this alteration with Coffin-Siris syndrome is unlikely. -
