This sequence change creates a premature translational stop signal (p.Gln425*) in the MYBPC3 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in MYBPC3 are known to be pathogenic (PMID: 19574547). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individuals with hypertrophic cardiomyopathy (PMID: 11815426, 18803133, 27532257). ClinVar contains an entry for this variant (Variation ID: 42517). For these reasons, this variant has been classified as Pathogenic. -

The Gln425X variant in MYBPC3 has been reported in 2 sporadic cases of late-onse t HCM (1 Polish and 1 of unspecified ancestry; Niimura 2002, Radzinski 2008). Th is variant has not been identified in large and broad European American and Afri can American populations by the NHLBI Exome Sequencing Project (http://evs.gs.wa shington.edu/EVS) though it remains possible that this variant is common in othe r populations. This variant leads to a premature termination codon at position 4 25, which is predicted to lead to a truncated or absent protein. Heterozygous lo ss-of-function of the MYBPC3 gene is an established disease mechanism in HCM. In summary, this variant meets our criteria to be classified as pathogenic (http:/ /pcpgm.partners.org/LMM) based on low frequency and the severity of the variant. -
