The c.4411T>C (p.S1471P) alteration is located in exon 33 (coding exon 32) of the MYO7A gene. This alteration results from a T to C substitution at nucleotide position 4411, causing the serine (S) at amino acid position 1471 to be replaced by a proline (P). for autosomal recessive Usher syndrome type I; however, its clinical significance for autosomal dominant and autosomal recessive MYO7A-related nonsyndromic hearing loss is uncertain This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This variant has been identified in the homozygous state and/or in conjunction with other MYO7A variants in individuals with a clinical diagnosis or suspected diagnosis of Usher syndrome; in at least one instance, the variants were identified in trans (Sodi, 2014; Bonnet, 2016; Sodi, 2018; Frustaci, 2021). This amino acid position is highly conserved in available vertebrate species. This alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, this alteration is classified as pathogenic. -

This sequence change replaces serine, which is neutral and polar, with proline, which is neutral and non-polar, at codon 1471 of the MYO7A protein (p.Ser1471Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with Usher syndrome (PMID: 25558175, 33835720; Invitae). ClinVar contains an entry for this variant (Variation ID: 43239). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MYO7A protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -
Rare genetic deafness Pathogenic:1
The Ser1471Pro variant in MYO7A has not been reported in the literature and was absent from >8,000 European American chromosomes from a broad population by the NHLBI Exome Sequencing Project (http://evs.gs.washington.edu/EVS/). However, it has previously been identified in trans with a pathogenic allele in two siblings with Usher Syndrome by our laboratory. In summary, this variant is likely patho genic, though additional studies are required to fully establish its clinical si gnificance. -
