Variant summary: CBL c.2589C>G (p.Asn863Lys) results in a non-conservative amino acid change located in the Ubiquitin-associated domain (IPR015940) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251452 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2589C>G has been reported in the literature in a newborn with congenital heart defect, however authors classified the variant as VUS (Hakami_2016). This report does not provide unequivocal conclusions about association of the variant with Noonan Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 26918529). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance. -
Variant classified as Uncertain Significance - Favor Benign. The Asn863Lys varia nt in CBL has not been reported in the literature nor previously identified by o ur laboratory. This residue is not conserved across species and computational an alyses (biochemical amino acid properties, conservation, AlignGVGD, PolyPhen2, a nd SIFT) do not suggest a high likelihood of impact to the protein. However, thi s information is not predictive enough to rule out pathogenicity. This variant has not been identified in large European American and African American populati ons by the NHLBI Exome Sequencing Project (http://evs.gs.washington.edu/EVS/), h owever an adjacent variant (c.2588A>G, p.Asn863Ser) has been identified in 0.05% (4/8590) of European American chromosomes (dbSNP rs146250423), suggesting this particular codon may be polymorphic in this population. In summary, additional information is needed to fully assess the clinical significance of the Asn863Lys variant. -
RASopathy Uncertain:1
This sequence change replaces asparagine, which is neutral and polar, with lysine, which is basic and polar, at codon 863 of the CBL protein (p.Asn863Lys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with congenital heart defect (PMID: 26918529). ClinVar contains an entry for this variant (Variation ID: 45207). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt CBL protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
