Genes affected
SOS1 (HGNC:11187): (SOS Ras/Rac guanine nucleotide exchange factor 1) This gene encodes a protein that is a guanine nucleotide exchange factor for RAS proteins, membrane proteins that bind guanine nucleotides and participate in signal transduction pathways. GTP binding activates and GTP hydrolysis inactivates RAS proteins. The product of this gene may regulate RAS proteins by facilitating the exchange of GTP for GDP. Mutations in this gene are associated with gingival fibromatosis 1 and Noonan syndrome type 4. [provided by RefSeq, Jul 2008]

ACMG classification

Classification was made for transcript

Our verdict: Uncertain_significance. The variant received 4 ACMG points.

PM1
In a hotspot region, there are 3 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 13 uncertain in NM_005633.4
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 2-39058683-G-C is Pathogenic according to our data. Variant chr2-39058683-G-C is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 45361.
BP4
Computational evidence support a benign effect (MetaRNN=0.35688463). . Strength limited to SUPPORTING due to the PP5.

This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 112 of the SOS1 protein (p.Pro112Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of Noonan syndrome (PMID: 21387466). ClinVar contains an entry for this variant (Variation ID: 45361). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SOS1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
