The p.Ser1404X variant in BRCA2 has been reported in at least 2 individuals with BRCA2-associated cancers (Guan 2015 PMID: 25151137; Dong 2018 PMID: 30039884), in 1 individual with a family history of breast and ovarian cancer (HBOC) that had undergone clinical genetic testing (Rebbeck 2018 PMID: 29446198) and was absent from large population studies. This variant is a single base deletion that creates a premature termination codon at position 1404, resulting in a nonsense variant which is predicted to lead to a truncated or absent protein. Loss of function of the BRCA2 gene is an established disease mechanism in autosomal dominant hereditary breast and ovarian cancer (HBOC). Moreover, this variant was classified as pathogenic on September 13, 2016 by the ClinGen-approved ENIGMA expert panel and several clinical labs (Variation ID 91815). In summary, this variant meets criteria to be classified as pathogenic for autosomal dominant HBOC. ACMG/AMP Criteria applied: PVS1, PM2_supporting, PS4_Supporting. -

Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 235862 control chromosomes. c.4211delC has been reported in the literature in individuals affected with Hereditary Breast and Ovarian Cancer (Rebbeck_2018). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Six clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

The c.4211delC pathogenic mutation, located in coding exon 10 of the BRCA2 gene, results from a deletion of one nucleotide at nucleotide position 4211, causing a translational frameshift with a predicted alternate stop codon (p.S1404*). A different alteration (c.4211C>G) resulting in a stop codon at the same amino acid position (p.S1404*) was previously reported in a 48 year old man with prostate cancer (Guan Y et al. Fam. Cancer. 2015 Mar;14:9-18). This alteration was identified in a large, worldwide study of BRCA1/2 mutation positive families (Rebbeck TR et al. Hum. Mutat. 2018 05;39:593-620). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
