Z-lines in striated muscle are a sign of this organization, representing the ends of actin thin filaments, titin, nebulin or nebulette and accessory proteins required for structure and function. This gene encodes a protein which interacts with nebulin in skeletal muscle or nebulette in cardiac muscle and alpha-actinin. In addition, this gene product can interact with a protein with the I-band indicating it has a regulatory as well as structural function. Alternative splicing results in multiple transcript variants. Given that there is no case data on this variant and that it is sufficiently rare in control populations, we consider this variant a variant of uncertain significance and we do not feel it is suitable for assessing risk in healthy relatives ("predictive genetic testing"). There is no case data available for this variant. The association between MYPN and cardiomyopathy is uncertain; in particular, the contribution of missense variants to cardiomyopathy: according to ExAC, this variant is intolerant of loss of function variants (pLI=0.07), but tolerant of missense variants (Z=-0.35) and synonymous variants (Z=-0.12). This patient has a missense variant In silico analysis with PolyPhen-2 predicts the variant to be probably damaging. The serine at codon 1256 is highly conserved across species. In total the variant has not been seen in at least 700 published controls, mostly of European descent (Duboscq-Bidot et al 2007, Meyer et al, 2013). This variant is listed in the Exome Aggregation Consortium dataset (http://exac.broadinstitute.org/), which currently includes variant calls on ~64,000 individuals of European, African, Latino and Asian descent (as of October 2016). This variant was present in 1 out of 60685 individuals. Specifically, it was present in 1 out of 5202 individuals of African descent. -
