The MSH2 c.1397A>G (p.His466Arg) variant has been reported in the published literature in an affected individual with colorectal cancer (PMID: 29212164 (2017)). It was also reported in an individual with breast cancer as well as in a reportedly healthy individual in a large scale breast cancer association study (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/MSH2)). The frequency of this variant in the general population, 0.00016 (5/30612 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

The p.H466R variant (also known as c.1397A>G), located in coding exon 9 of the MSH2 gene, results from an A to G substitution at nucleotide position 1397. The histidine at codon 466 is replaced by arginine, an amino acid with highly similar properties. This alteration has been reported in a patient with rectal and uterine cancer at age 40 from a cohort of 1,046 familial colorectal cancer cases (Raskin L et al. Oncotarget, 2017 Nov;8:93450-93463). This variant was also reported in 1/60,466 breast cancer cases and in 1/53,461 controls (Dorling et al. N Engl J Med. 2021 02;384:428-439). In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was determined to be functionally neutral (Jia X et al. Am J Hum Genet, 2021 Jan;108:163-175). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
