Genes affected
ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Mutations in this gene are associated with ataxia telangiectasia, an autosomal recessive disorder. [provided by RefSeq, Aug 2010]

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsJul 19, 2023The p.G757R variant (also known as c.2269G>A), located in coding exon 14 of the ATM gene, results from a G to A substitution at nucleotide position 2269. The glycine at codon 757 is replaced by arginine, an amino acid with dissimilar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
Uncertain significance, criteria provided, single submitterclinical testingColor Diagnostics, LLC DBA Color HealthOct 17, 2023This missense variant replaces glycine with arginine at codon 757 of the ATM protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been performed for this variant. In a large international case-control study, this variant was reported in 4/60462 breast cancer cases and 3/53458 controls (PMID: 33471991). This variant was also reported in an individual referred for hereditary cancer genetic testing (PMID: 31159747) and an individual with a strong family history of breast cancer (PMID: 34204722). This variant has been identified in 4/282484 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Uncertain significance, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpDec 18, 2023This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 757 of the ATM protein (p.Gly757Arg). This variant is present in population databases (rs587779819, gnomAD 0.002%). This missense change has been observed in individual(s) with a personal or family history of breast and/or ovarian cancer (PMID: 31159747, 32830346, 34204722, 36099812). ClinVar contains an entry for this variant (Variation ID: 127345). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Uncertain significance, criteria provided, single submitterclinical testingGeneDxDec 05, 2022Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; In silico analysis is inconclusive as to whether the variant alters gene splicing. In the absence of RNA/functional studies, the actual effect of this sequence change is unknown; Observed in individuals with a personal and/or family history of breast and/or ovarian cancer, as well as in unaffected controls (Tsaousis et al., 2019; Dorling et al., 2021); This variant is associated with the following publications: (PMID: 31159747, 33471991) -

Uncertain significance, criteria provided, single submitterclinical testingWomen's Health and Genetics/Laboratory Corporation of America, LabCorpAug 04, 2023Variant summary: ATM c.2269G>A (p.Gly757Arg) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251080 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2269G>A has been reported in the literature in individuals affected with or at risk for hereditary cancers (e.g. Tsaousis_2019, Smith_2021, Fonfria_2021) and one individual with primary ovarian insufficiency (e.g. Heddar_2022). In all cases, the authors classified the alteration as a VUS. Therefore, these reports do not provide unequivocal conclusions about association of the variant with Ataxia-Telangiectasia (AT) and/or AT-related cancers. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 34204722, 36099812, 32830346, 31159747). Nine submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
