This sequence change falls in intron 8 of the BRCA2 gene. It does not directly change the encoded amino acid sequence of the BRCA2 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. ClinVar contains an entry for this variant (Variation ID: 141090). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in skipping of exon 8 and introduces a premature termination codon (Invitae). The resulting mRNA is expected to undergo nonsense-mediated decay. For these reasons, this variant has been classified as Pathogenic. -
Variant summary: BRCA2 c.681+2dupT alters a conserved nucleotide located close to a canonical splice site and therefore could affect mRNA splicing, leading to a significantly altered protein sequence. Several computational tools predict a significant impact on normal splicing: Four predict the variant abolishes a 5' splicing donor site. At least one publication reports experimental evidence that this variant affects mRNA splicing (Karam_2019). The variant was absent in 243714 control chromosomes. c.681+2dupT has been reported in the literature in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (Petridis_2019). The following publications have been ascertained in the context of this evaluation (PMID: 31642931, 31263054). Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as pathogenic/likely pathogenic. Based on the evidence outlined above, the variant was classified as likely pathogenic. -

Although this prediction has not been confirmed in published RNA studies, this variant is expected to result in an absent or disrupted protein product. This variant has been reported in 1 individual affected with breast cancer (Color internal data). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of BRCA2 function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Likely Pathogenic. -
The c.681+2dupT intronic variant results from a duplication of one nucleotide two nucleotides after coding exon 6 of the BRCA2 gene. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and will result in the creation or strengthening of a novel splice donor site. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. However, RNA studies have demonstrated that this alteration results a combination of in-frame transcript and out of frame transcripts (Ambry internal data). Data show that one of these observed transcripts, an in-frame deletion of 68 amino acids, is able to rescue Brca2-null mouse embryonic stem cell survival defects and has near normal homology directed DNA repair function (Mesman RLS et al. Genet Med, 2020 08;22:1355-1365). Therefore, the clinical impact of this abnormal splicing is unknown at this time. Based on the available evidence, the clinical significance of this variant remains unclear. -

Therefore, the c.681+2dup variant in the BRCA2 gene has been classified as likely pathogenic. -
