Genes affected
NF1 (HGNC:7765): (neurofibromin 1) This gene product appears to function as a negative regulator of the ras signal transduction pathway. Mutations in this gene have been linked to neurofibromatosis type 1, juvenile myelomonocytic leukemia and Watson syndrome. The mRNA for this gene is subject to RNA editing (CGA>UGA->Arg1306Term) resulting in premature translation termination. Alternatively spliced transcript variants encoding different isoforms have also been described for this gene. [provided by RefSeq, Jul 2008]

Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Oct 30, 2015 | The p.K2614R variant (also known as c.7841A>G), located in coding exon 53 of the NF1 gene, results from an A to G substitution at nucleotide position 7841. The lysine at codon 2614 is replaced by arginine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.003% (greater than 110000alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive.Since supporting evidence is limited at this time, the clinical significance of p.K2614Rremains unclear. - |

Uncertain significance, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Aug 01, 2023 | Variant summary: NF1 c.7778A>G (p.Lys2593Arg) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 5.1e-05 in 276090 control chromosomes (gnomAD, Momozawa_2018). This frequency is not significantly higher than estimated for a pathogenic variant in NF1 causing Neurofibromatosis Type 1 (5.1e-05 vs 0.00021), allowing no conclusion about variant significance. c.7778A>G has been reported in the literature in individuals affected with Breast Cancer (Dorling_2021) and Biliary Tract Cancer (Okawa_2023) without evidence for causality. These reports do not provide unequivocal conclusions about association of the variant with Neurofibromatosis Type 1. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 30287823, 33471991, 36243179). Five ClinVar submitters have assessed the variant since 2014: two classified the variant as uncertain significance and three as likely benign. Based on the evidence outlined above, the variant was classified as uncertain significance. - |

Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | Sep 23, 2022 | In silico analysis supports that this missense variant does not alter protein structure/function; Has not been observed in patients with NF1-related features to our knowledge, but has been reported in a control cohort for a case-control study of breast cancer (Momozawa et al., 2018); This variant is associated with the following publications: (PMID: 30287823, 25486365) - |

Hereditary cancer-predisposing syndrome;CN230736:Cardiovascular phenotype Benign:1
Likely benign, criteria provided, single submitter | clinical testing | Ambry Genetics | Oct 21, 2020 | This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. - |
