The lysine at codon 2614 is replaced by arginine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.003% (greater than 110000alleles tested) in our clinical cohort.This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive.Since supporting evidence is limited at this time, the clinical significance of p.K2614Rremains unclear. - |

Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 5.1e-05 in 276090 control chromosomes (gnomAD, Momozawa_2018). This frequency is not significantly higher than estimated for a pathogenic variant in NF1 causing Neurofibromatosis Type 1 (5.1e-05 vs 0.00021), allowing no conclusion about variant significance. c.7778A>G has been reported in the literature in individuals affected with Breast Cancer (Dorling_2021) and Biliary Tract Cancer (Okawa_2023) without evidence for causality. These reports do not provide unequivocal conclusions about association of the variant with Neurofibromatosis Type 1. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 30287823, 33471991, 36243179). Five ClinVar submitters have assessed the variant since 2014: two classified the variant as uncertain significance and three as likely benign.
