Variant summary
Our verdict is Likely benign. The variant received -1 ACMG points: 0P and 1B. BP4
The NM_005591.4(MRE11):c.482A>G(p.Lys161Arg) variant causes a missense change. The variant allele was found at a frequency of 0.0000217 in 1,613,610 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. K161T) has been classified as Uncertain significance.

Variant summary: MRE11A c.482A>G (p.Lys161Arg) results in a conservative amino acid change located in the Mre11 nuclease, N-terminal metallophosphatase domain (IPR041796) of the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251036 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.482A>G has been reported in the literature in individuals affected with Breast Cancer, as well as unaffected controls (Dorling_2021, Felicio_2021, de Oliveira_2022). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 33471991, 35534704, 33326660). ClinVar contains an entry for this variant (Variation ID: 142190). Based on the evidence outlined above, the variant was classified as uncertain significance. -

Hereditary cancer-predisposing syndrome Uncertain:1
The p.K161R variant (also known as c.482A>G), located in coding exon 5 of the MRE11A gene, results from an A to G substitution at nucleotide position 482. The lysine at codon 161 is replaced by arginine, an amino acid with highly similar properties. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
Ataxia-telangiectasia-like disorder Uncertain:1
This sequence change replaces lysine, which is basic and polar, with arginine, which is basic and polar, at codon 161 of the MRE11 protein (p.Lys161Arg). This variant is present in population databases (rs587782294, gnomAD 0.003%). This missense change has been observed in individual(s) with breast cancer (PMID: 35534704). ClinVar contains an entry for this variant (Variation ID: 142190). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
