This missense variant replaces arginine with glutamine at codon 1005 of the MSH6 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with sporadic colon cancer (UMD: http://139.124.156.133/4D_molecules/UMD012708.html, PMID: 10612827). This variant has been identified in 2/241392 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.R1005Q variant (also known as c.3014G>A), located in coding exon 4 of the MSH6 gene, results from a G to A substitution at nucleotide position 3014. The arginine at codon 1005 is replaced by glutamine, an amino acid with highly similar properties. This alteration was observed homozygous in an individual whose clinical features are consistent with constitutional mismatch repair deficiency (CMMRD) syndrome (Ambry internal data). However, this alteration was also observed homozygous in an individual from the same family with no reported features of CMMRD. In an assay testing MSH6 function, this variant showed a functionally indeterminant result (van der Werf't Lam AS et al. Exp Mol Pathol, 2024 Dec;140:104940). This variant has been identified in probands whose Lynch syndrome-associated tumor demonstrated high microsatellite instability and/or loss of MSH6 or MSH2/MSH6 expression by immunohistochemistry (Li S et al. J. Med. Genet. 2020 Jan;57:62-69; van der Werf't Lam AS et al. Exp Mol Pathol, 2024 Dec;140:104940; Ambry internal data; external laboratory communication). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

Variant summary: MSH6 c.3014G>A (p.Arg1005Gln) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.8e-06 in 209994 control chromosomes. c.3014G>A has been reported in the literature in individuals affected with colorectal cancer (example, Li_2020). A recent study reporting tumor characteristic likelihood ratio (TCLR) in combination with in silico predictors and a multifactorial variant prediction model that included allele frequency, co-occurrence, co-segregation, clinical and family history information, classified this variant as uncertain significance (Li_2020). It has also been observed in individuals with clinical features of Lynch Syndrome tested at our laboratory (Internal data). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 31391288, 11807791). ClinVar contains an entry for this variant (Variation ID: 142232). Based on the evidence outlined above, the variant was classified as likely pathogenic. -

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 1005 of the MSH6 protein (p.Arg1005Gln). This variant is present in population databases (rs587782324, gnomAD 0.009%). This missense change has been observed in individuals with clinical features of Lynch syndrome (internal data). Invitae Evidence Modeling of clinical and family history, age, sex, and reported ancestry of multiple individuals with this MSH6 variant has been performed. This variant is expected to be pathogenic with a positive predictive value of at least 99%. This is a validated machine learning model that incorporates the clinical features of 1,370,736 individuals referred to our laboratory for MSH6 testing. ClinVar contains an entry for this variant (Variation ID: 142232). Invitae Evidence Modeling incorporating data from in vitro experimental studies (internal data) indicates that this missense variant is expected to disrupt MSH6 function with a positive predictive value of 95%.
