Genes affected
PALB2 (HGNC:26144): (partner and localizer of BRCA2) This gene encodes a protein that may function in tumor suppression. This protein binds to and colocalizes with the breast cancer 2 early onset protein (BRCA2) in nuclear foci and likely permits the stable intranuclear localization and accumulation of BRCA2. Likely benign, criteria provided, single submitterclinical testingMyriad Genetics, Inc.Mar 31, 2023This variant is considered likely benign. This variant is strongly associated with less severe personal and family histories of cancer, typical for individuals without pathogenic variants in this gene [PMID: 25085752]. -
Uncertain significance, criteria provided, single submitterclinical testingInvitaeJan 25, 2024This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 873 of the PALB2 protein (p.Ser873Arg). This variant is present in population databases (rs587782387, gnomAD 0.0009%). This missense change has been observed in individual(s) with pancreatic adenocarcinoma and/or triple-negative breast cancer (PMID: 25452441, 28767289). ClinVar contains an entry for this variant (Variation ID: 142328). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PALB2 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Uncertain significance, criteria provided, single submitterclinical testingColor Diagnostics, LLC DBA Color HealthAug 23, 2022This missense variant replaces serine with arginine at codon 873 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in at least 3 individuals affected with breast cancer, one individual affected with pancreatic cancer with family history of breast cancer and one unaffected individual (PMID: 25452441, 28767289, 33471991; Leiden Open Variation Database DB-ID PALB2_010315). This variant has been identified in 1/251486 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsMar 27, 2023The p.S873R variant (also known as c.2619T>G), located in coding exon 7 of the PALB2 gene, results from a T to G substitution at nucleotide position 2619. The serine at codon 873 is replaced by arginine, an amino acid with dissimilar properties. This alteration has been detected in 1/1824 patients with triple negative breast cancer who were unselected for a family history of breast or ovarian cancer (Couch FJ et al. J Clin Oncol, 2015 Feb;33:304-11). This alteration has also been reported in at least one subject in a study of 13087 breast cancer cases and 5488 control individuals in the UK (Decker B et al. J Med Genet, 2017 11;54:732-741). Additionally, this alteration has been identified in multiple individuals diagnosed with pancreatic cancer (Shindo K et al. J Clin Oncol, 2017 Oct;35:3382-3390; Hu H et al. J Am Coll Surg, 2020 Nov;231:527-535.e14). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
not specified Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingWomen's Health and Genetics/Laboratory Corporation of America, LabCorpFeb 25, 2019Variant summary: PALB2 c.2619T>G (p.Ser873Arg) results in a non-conservative amino acid change located in the WD40 domain (IPR031920) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.1e-06 in 246262 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2619T>G has been reported in the literature in an individual affected with triple-negative breast cancer (Couch 2015) and in another individual with pancreatic cancer (Shindo 2017). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast and Ovarian Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
Familial ovarian cancer Uncertain:1
Uncertain significance, no assertion criteria providedclinical testingDepartment of Pathology and Laboratory Medicine, Sinai Health System-The PALB2 p.Ser873Arg variant was identified in 1 of 3648 proband chromosomes (frequency: 0.000) from American individuals with triple negative breast cancer (Couch 2015). The variant was also identified in dbSNP (ID: rs587782387) “With Uncertain significance allele”, ClinVar (classified as uncertain significance by Ambry Genetics, Invitae and Quest Diagnostics Nichols Institute San Juan Capistrano), Clinvitae (2x), but was not identified in the Cosmic and Zhejiang Colon Cancer Database. The variant was identified in control databases in 1 of 246262 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017). Breakdown of the observations by population include European Non-Finnish in 1 of 111714 chromosomes (freq: 0.000009), and not observed in the African, Other, Latino, Ashkenazi Jewish, East Asian, European Finnish, and South Asian populations. The p.Ser873 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and 3 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) predict a greater than 10% difference in splicing. However, this information is not predictive enough to assume pathogenicity. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. -
