This missense change has been observed in individual(s) with pancreatic adenocarcinoma and/or triple-negative breast cancer (PMID: 25452441, 28767289). This missense change has been observed to be homozygous, hemizygous or homoplasmic in an individual who did not have the expected clinical features for that genetic result (internal data). ClinVar contains an entry for this variant (Variation ID: 142328). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt PALB2 protein function with a positive predictive value of 80%. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in at least 3 individuals affected with breast cancer, one individual affected with pancreatic cancer with family history of breast cancer and one unaffected individual (PMID: 25452441, 28767289, 33471991; Leiden Open Variation Database DB-ID PALB2_010315). This variant has been identified in 1/251486 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Variant summary: PALB2 c.2619T>G (p.Ser873Arg) results in a non-conservative amino acid change located in the WD40 domain (IPR031920) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251486 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2619T>G has been observed in individuals affected with breast cancer or pancreatic cancer without evidence of cosegregation with disease (e.g., Couch_2015, Shindo 2017, Dorling_2021). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The PALB2 p.Ser873Arg variant was identified in 1 of 3648 proband chromosomes (frequency: 0.000) from American individuals with triple negative breast cancer (Couch 2015). The variant was also identified in dbSNP (ID: rs587782387) “With Uncertain significance allele”, ClinVar (classified as uncertain significance by Ambry Genetics, Invitae and Quest Diagnostics Nichols Institute San Juan Capistrano), Clinvitae (2x), but was not identified in the Cosmic and Zhejiang Colon Cancer Database. The variant was identified in control databases in 1 of 246262 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017). Breakdown of the observations by population include European Non-Finnish in 1 of 111714 chromosomes (freq: 0.000009), and not observed in the African, Other, Latino, Ashkenazi Jewish, East Asian, European Finnish, and South Asian populations. The p.Ser873 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and 3 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) predict a greater than 10% difference in splicing.
