ATP7B (HGNC:870): (ATPase copper transporting beta) This gene is a member of the P-type cation transport ATPase family and encodes a protein with several membrane-spanning domains, an ATPase consensus sequence, a hinge domain, a phosphorylation site, and at least 2 putative copper-binding sites. This protein is a monomer, and functions as a copper-transporting ATPase which exports copper out of the cells, such as the efflux of hepatic copper into the bile. Alternate transcriptional splice variants, encoding different isoforms with distinct cellular localizations, have been characterized. Mutations in this gene have been associated with Wilson disease which is characterized by copper accumulation. [provided by RefSeq, Dec 2019]

Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ATP7B protein function with a positive predictive value of 80%. Variant summary: ATP7B c.3188C>T (p.Ala1063Val) results in a non-conservative amino acid change located in the P-type ATPase, haloacid dehalogenase domain (IPR044492) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 249356 control chromosomes. c.3188C>T has been reported in the literature in multiple individuals affected with Wilson Disease (Loudianos_1999, Weiss_2010, AbdelGhaffar_2011). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 21682854, 10544227, 20517649). Four submitters have cited clinical-significance assessments for this variant to ClinVar after 2014 and classified as pathogenic/likely pathogenic (n=2) and VUS (n=2). Based on the evidence outlined above, the variant was classified as pathogenic. -

The p.Ala1063Val variant in ATP7B has been reported in several individuals with Wilson disease, including at least 2 homozygotes and 3 compound heterozygous individuals (Abdelghaffar 2008, Bost 2012, Cheng 2017, Coffey 2013, Gojova 2008, Loudianos 1999, Merle 2010, Nagasaka 2012, Nanji 1997, Nemeth 2016, Todorov 2016, Vrabelova 2005, Zong 2015). This variant is absent from large population studies. Computational prediction tools and conservation analyses suggest an impact to the protein. In summary, although additional studies are required to fully establish its clinical significance, this variant meets criteria to be classified as likely pathogenic for autosomal recessive Wilson disease. ACMG/AMP criteria applied: PM3_Strong, PM2, PP3, PP4. -
