Variant summary
Our verdict is Pathogenic. The variant received 12 ACMG points: 12P and 0B. PM1PM2PM5PP3_StrongPP5_Moderate
The NM_133433.4(NIPBL):c.5464G>A(p.Asp1822Asn) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D1822G) has been classified as Likely pathogenic.

Cornelia de Lange syndrome 1 Pathogenic:1
This variant disrupts the p.Asp1822 amino acid residue in NIPBL. Other variant(s) that disrupt this residue have been observed in individuals with NIPBL-related conditions (PMID: 28425213, 29620724), which suggests that this may be a clinically significant amino acid residue. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NIPBL protein function. This missense change has been observed in individuals with Cornelia de Lange syndrome (PMID: 28425213; Invitae). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces aspartic acid, which is acidic and polar, with asparagine, which is neutral and polar, at codon 1822 of the NIPBL protein (p.Asp1822Asn). -
