Genes affected
NSD1 (HGNC:14234): (nuclear receptor binding SET domain protein 1) This gene encodes a protein containing a SET domain, 2 LXXLL motifs, 3 nuclear translocation signals (NLSs), 4 plant homeodomain (PHD) finger regions, and a proline-rich region. The encoded protein enhances androgen receptor (AR) transactivation, and this enhancement can be increased further in the presence of other androgen receptor associated coregulators. This protein may act as a nucleus-localized, basic transcriptional factor and also as a bifunctional transcriptional regulator. Mutations of this gene have been associated with Sotos syndrome and Weaver syndrome. One version of childhood acute myeloid leukemia is the result of a cryptic translocation with the breakpoints occurring within nuclear receptor-binding Su-var, enhancer of zeste, and trithorax domain protein 1 on chromosome 5 and nucleoporin, 98-kd on chromosome 11. Multiple transcript variants encoding distinct isoforms have been identified for this gene. [provided by RefSeq, Sep 2018]

This sequence change creates a premature translational stop signal (p.Arg1473*) in the NSD1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in NSD1 are known to be pathogenic (PMID: 12464997, 14571271, 15942875, 16247291). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with NSD1-related conditions (PMID: 12464997, 28475857). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 159331). For these reasons, this variant has been classified as Pathogenic. -

This sequence variant is a single nucleotide substitution (C>T) at position 4417 of the NSD1 gene that results in the generation of an early termition codon at residue 1473 of the NSD1 protein. This variant is predicted to result in a non-functiol allele through either the expression of a truncated protein or a loss of NSD1 expression due to nonsense mediated decay. This is a previously reported variant (ClinVar) that has been observed in multiple individiuals affected with Sotos Syndrome (PMID: 15942875). This variant is absent from control population datasets (gnomAD database 0/~250,000 alleles). Based upon the evidence, we consider this variant to be pathogenic. ACMG Criteria: PM2, PS2, PVS1 -

The NSD1 c.4417C>T variant is predicted to result in premature protein termination (p.Arg1473*). This variant has been reported in multiple individual with Sotos syndrome (see for example Douglas et al. 2003. PubMed ID: 12464997). This variant has not been reported in a large population database (http://gnomad.broadinstitute.org), indicating this variant is rare. Nonsense variants in NSD1 are expected to be pathogenic. This variant is interpreted as pathogenic. -
