When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. The threonine at codon 552 is replaced by proline, an amino acid with highly similar properties. This alteration was detected in a family meeting Amsterdam criteria in which the proband had a microsatellite unstable (MSI-H) colon tumor that showed normal MLH1, MSH2, and MSH6 protein expression (Wagner A et al. Am J Hum Genet. 2003 May;72(5):1088-100). This alteration was also detected in a patient with colon cancer who had at least 2 family members with colorectal or endometrial cancer in two generations (Stojcev Z et al. Acta Biochim Pol. 2013;60(2):195-8). In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was reported to be functionally deleterious (Jia X et al. Am J Hum Genet, 2021 01;108:163-175). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
