Variant summary

Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong

The NM_000251.3(MSH2):c.1968C>A(p.Tyr656Ter) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. Y656Y) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Genes affected
MSH2 (HGNC:7325): (mutS homolog 2) This locus is frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. Two transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Apr 2012]

Lynch syndrome 1 Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingMyriad Genetics, Inc.Aug 04, 2023This variant is considered pathogenic. This variant creates a termination codon and is predicted to result in premature protein truncation. -
not provided Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingGeneDxJul 20, 2015This pathogenic variant is denoted MSH2 c.1968C>A at the cDNA level and p.Tyr656Ter (Y656X) at the protein level. The substitution creates a nonsense variant, which changes a Tyrosine to a premature stop codon (TAC>TAA), and is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. This variant has been reported in at least one individual with a personal and/or family history suspicious for Lynch syndrome (Yurgelun 2015). In addition, different nucleotide substitutions resulting in the same nonsense variant, MSH2 Tyr656Ter, have also been reported in families with clinical histories suspicious for Lynch syndrome (Kohonen-Corish 1996, Scott 2001, Taylor 2003, Mangold 2005, Kurzawski 2006, Loizidou 2014). we consider this variant to be pathogenic. -
Hereditary nonpolyposis colorectal neoplasms Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingInvitaeJan 09, 2024This sequence change creates a premature translational stop signal (p.Tyr656*) in the MSH2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in MSH2 are known to be pathogenic (PMID: 15849733, 24362816). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with clinical features of Lynch syndrome (PMID: 8808596, 15849733, 25133505, 25980754, 28449805). ClinVar contains an entry for this variant (Variation ID: 419473). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic. -
Hereditary cancer-predisposing syndrome Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsAug 29, 2019The p.Y656* pathogenic mutation (also known as c.1968C>A), located in coding exon 12 of the MSH2 gene, results from a C to A substitution at nucleotide position 1968. This changes the amino acid from a tyrosine to a stop codon within coding exon 12. This alteration was identified in a cohort of 1260 individuals undergoing panel testing for Lynch syndrome due to having a diagnosis of a Lynch-associated cancer and/or polyps. (Yurgelun MB et al. Gastroenterology, 2015 Sep;149:604-13.e20). Another nucleotide substitution at position 1968 (c.1968C>G), resulting in the same amino acid substitution, has been described in multiple HNPCC families (Kohonen-Corish M et al. Am. J. Hum. Genet. 1996 Oct; 59(4):818-24; Mangold E et al. Int. J. Cancer 2005 Sep; 116(5):692-702; Loizidou MA et al. PLoS ONE, 2014 Aug;9:e105501; Sunga AY et al. Cancer Genet, 2017 04;212-213:1-7). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
