Genes affected
GLB1 (HGNC:4298): (galactosidase beta 1) This gene encodes a member of the glycosyl hydrolase 35 family of proteins. Alternative splicing results in multiple transcript variants, at least one of which encodes a preproprotein that is proteolytically processed to generate the mature lysosomal enzyme. This enzyme catalyzes the hydrolysis of a terminal beta-linked galactose residue from ganglioside substrates and other glycoconjugates. Mutations in this gene may result in GM1-gangliosidosis and Morquio B syndrome. [provided by RefSeq, Nov 2015]

This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 591 of the GLB1 protein (p.Tyr591Cys). This variant is present in population databases (rs72555371, gnomAD 0.003%). This missense change has been observed in individual(s) with GM1-gangliosidosis (PMID: 10737981). ClinVar contains an entry for this variant (Variation ID: 948). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt GLB1 protein function. This variant disrupts the p.Tyr591 amino acid residue in GLB1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 10737981). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -

The variant is observed at an extremely low frequency in the gnomAD v2.1.1 dataset (total allele frequency: 0.001%). Predicted Consequence/Location: Missense variant In silico tool predictions suggest damaging effect of the variant on gene or gene product (REVEL: 0.93; 3Cnet: 0.92). Same nucleotide change resulting in same amino acid change has been previously reported to be associated with GLB1 related disorder (ClinVar ID: VCV000000948 /PMID: 10737981).Different missense changes at the same codon (p.Tyr591Asn, p.Tyr591His, p.Tyr591Ser) have been reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000000947, VCV001472090, VCV002151971 /PMID: 10737981). Therefore, this variant is classified as Likely pathogenic according to the recommendation of ACMG/AMP guideline. -
