Genes affected
COL1A1 (HGNC:2197): (collagen type I alpha 1 chain) This gene encodes the pro-alpha1 chains of type I collagen whose triple helix comprises two alpha1 chains and one alpha2 chain. Type I is a fibril-forming collagen found in most connective tissues and is abundant in bone, cornea, dermis and tendon. Mutations in this gene are associated with osteogenesis imperfecta types I-IV, Ehlers-Danlos syndrome type VIIA, Ehlers-Danlos syndrome Classical type, Caffey Disease and idiopathic osteoporosis. Reciprocal translocations between chromosomes 17 and 22, where this gene and the gene for platelet-derived growth factor beta are located, are associated with a particular type of skin tumor called dermatofibrosarcoma protuberans, resulting from unregulated expression of the growth factor. Two transcripts, resulting from the use of alternate polyadenylation signals, have been identified for this gene. [provided by R. Dalgleish, Feb 2008]

The variant is not observed in the gnomAD v2.1.1 dataset. Predicted Consequence/Location: Frameshift: predicted to result in a loss or disruption of normal protein function through nonsense-mediated decay (NMD) or protein truncation. Multiple pathogenic variants are reported downstream of the variant. The variant has been reported at least twice as pathogenic with clinical assertions and evidence for the classification (ClinVar ID: VCV000035925 /PMID: 16879195). Therefore, this variant is classified as Pathogenic according to the recommendation of ACMG/AMP guideline. -

This variant is predicted to substitute a glycine residue by a valine residue, introduce a frameshift resulting in a premature stop codon 71 amino acids downstream. Variants predicted to introduce termination codons lead to degradation of the affected transcript and haploinsufficiency of the alpha 1 chain of collagen type I, which is a typical cause of OI type I. This variant is absent from the Genome Aggregation Database (v2.1.1), indicating it is rare. This variant has been reported in the literature (PMID: 25944380) as a cause of osteogenesis imperfecta. -

This sequence change creates a premature translational stop signal (p.Gly194Valfs*71) in the COL1A1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in COL1A1 are known to be pathogenic (PMID: 7942841, 9295084, 9443882). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with osteogenesis imperfecta (PMID: 16879195, 27510842). ClinVar contains an entry for this variant (Variation ID: 35925). For these reasons, this variant has been classified as Pathogenic. -

Variant summary: COL1A1 c.579delT (p.Gly194ValfsX71) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, however current evidence is not sufficient to establish loss of function as a mechanism for disease. The frequency data for this variant in gnomAD is considered unreliable, as metrics indicate poor data quality at this position. c.579delT has been observed in individuals affected with Osteogenesis Imperfecta (Venturi_2006, Swinnen_2009). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 16879195, 19358256). ClinVar contains an entry for this variant (Variation ID: 35925). Based on the evidence outlined above, the variant was classified as pathogenic. -
