Genes affected
ELN (HGNC:3327): (elastin) This gene encodes a protein that is one of the two components of elastic fibers. Elastic fibers comprise part of the extracellular matrix and confer elasticity to organs and tissues including the heart, skin, lungs, ligaments, and blood vessels. The encoded protein is rich in hydrophobic amino acids such as glycine and proline, which form mobile hydrophobic regions bounded by crosslinks between lysine residues. Degradation products of the encoded protein, known as elastin-derived peptides or elastokines, bind the elastin receptor complex and other receptors and stimulate migration and proliferation of monocytes and skin fibroblasts. Elastokines can also contribute to cancer progression. Deletions and mutations in this gene are associated with supravalvular aortic stenosis (SVAS) and autosomal dominant cutis laxa. ACMG classification
Classification made for transcript
Verdict is Pathogenic. Variant got 12 ACMG points.
PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.04091954 fraction of the gene. Cryptic splice site detected, with MaxEntScore 9.4, offset of -44, new splice context is: gccatcctgcctgtcctcAGgag. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in inframe change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 7-74051748-A-G is Pathogenic according to our data. Variant chr7-74051748-A-G is described in ClinVar as [Pathogenic]. Clinvar id is 163387.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Pathogenic, no assertion criteria provided | literature only | OMIM | Jul 01, 1997 | - - |
Pathogenic, criteria provided, single submitter | clinical testing | Laboratory for Molecular Medicine, Mass General Brigham Personalized Medicine | May 27, 2011 | The 800-2A>G variant has been reported in 2 individuals with SVAS and was absent from more than 750 control chromosomes (Li 1997). In addition, this variant was observed to segregate with disease in 4 affected family members. Furthermore, t his variant is predicted to cause abnormal splicing because the nucleotide subst itution occurs in the highly conserved splice consensus sequence. Splice-site al terations are a reported cause of SVAS in the ELN gene (Human Gene Mutation Data base, HGMD). In summary, this variant meets our pathogenicity criteria (http://p cpgm.partners.org/LMM). - |
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Oct 04, 2023 | This sequence change affects an acceptor splice site in intron 15 of the ELN gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in ELN are known to be pathogenic (PMID: 11175284). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individuals with supravalvular aortic stenosis (PMID: 9215670). ClinVar contains an entry for this variant (Variation ID: 163387). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic. - |
