The p.Asn200Tyr variant in PTPN11 has been reported in one individual with Noona n syndrome and multiple giant cell lesions and in her mother with Noonan syndrom e without multiple giant cell lesions (Carapito 2014). This variant was reported to have occurred de novo in the proband's mother. It has also been identified i n two individuals with clinical features of Noonan syndrome by our laboratory. T his variant was absent from large population studies. In summary, this variant m eets our criteria to be classified as pathogenic (http://www.partners.org/person alizedmedicine/LMM). -

The PTPN11 c.598A>T variant is predicted to result in the amino acid substitution p.Asn200Tyr. This variant has been reported, often arising de novo, in individuals with Noonan syndrome (see for examples: Carapito et al. 2014. PubMed ID: 24225993; Ferriero et al. 2020. PubMed ID: 33042901; Dufke et al. 2022. PubMed ID: 35574990). This variant is reported in 0.12% of alleles in individuals of Latino descent in gnomAD; however, the total allele number for this dataset is relatively small and larger updated datasets report a highest allele frequency of of 0.002% (https://gnomad.broadinstitute.org/variant/12-112454636-A-T?dataset=gnomad_r4). Given the evidence, we interpret this variant as pathogenic. -

This variant is present in population databases (rs727503381, gnomAD 0.1%). This missense change has been observed in individual(s) with clinical features of PTPN11-related conditions and/or Noonan syndrome (PMID: 24225993, 30417923, 31941532, 33042901, 33057194, 35574990, 35982159). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 164998). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on PTPN11 protein function. For these reasons, this variant has been classified as Pathogenic. -
