Rare genetic deafness Pathogenic:1
The Arg457Trp variant in CDH23 has been reported in 1/188 individuals with Usher syndrome and 2/878 control chromosomes (Le Quesne Stabej 2012). Although this variant has been seen in controls, its frequency is low enough to be consistent with a recessive carrier frequency. In addition, Computational analyses (biochem ical amino acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) sugges t that the Arg457Trp variant may impact the protein, though this information alo ne is not predictive enough to determine pathogenicity. Furthermore, the identif ication of this variant in trans with a pathogenic CDH23 variant in two affected children in this family increases the likelihood that this variant is pathogeni c. In summary, based on the identification of this variant in two affected famil y members with a second pathogenic CDH23 variant, this variant is likely to be p athogenic. -

Predicted Consequence/Location: The majority of the known disease-causing variants of this gene are variants expected to result in premature termination of the protein. Premature termination of the protein is a common disease-causing mechanism for this gene. Damaging effect on gene or gene product predicted by in silico programs is uncertain [REVEL: 0.42 (damaging >=0.6, benign <0.4), 3Cnet: 0.05 (damaging >=0.6, benign <0.15)] Same nucleotide change resulting in same amino acid change has been previously reported to be associated with CDH23 related disorder (ClinVar ID: VCV000178675 /PMID: 31589614). However, the evidence of pathogenicity is insufficient at this time. Therefore, this variant is classified as VUS according to the recommendation of ACMG/AMP guideline. -
