Variant summary

Our verdict is Uncertain significance. Variant got 0 ACMG points: 3P and 3B. PM2PP2BP4_ModerateBP6

The NM_014000.3(VCL):c.1337C>G(p.Ala446Gly) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a benign outcome for this variant. 15/21 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars).

Genes affected
VCL (HGNC:12665): (vinculin) Vinculin is a cytoskeletal protein associated with cell-cell and cell-matrix junctions, where it is thought to function as one of several interacting proteins involved in anchoring F-actin to the membrane. Defects in VCL are the cause of cardiomyopathy dilated type 1W. Dilated cardiomyopathy is a disorder characterized by ventricular dilation and impaired systolic function, resulting in congestive heart failure and arrhythmia. Multiple alternatively spliced transcript variants have been found for this gene, but the biological validity of some variants has not been determined. [provided by RefSeq, Jul 2008]

ACMG classification

Classification made for transcript

Verdict is Uncertain_significance. Variant got 0 ACMG points.

PM2
Very rare variant in population databases, with high coverage;
PP2
Missense variant where missense usually causes diseases, VCL
BP4
Computational evidence support a benign effect (MetaRNN=0.14100772).
BP6
Variant 10-74090183-C-G is Benign according to our data. Variant chr10-74090183-C-G is described in ClinVar as [Conflicting_classifications_of_pathogenicity]. Clinvar id is 179157.We mark this variant Likely_benign, oryginal submissions are: {Uncertain_significance=1, Likely_benign=1}.

Dilated cardiomyopathy 1W Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingInvitaeApr 11, 2019This variant has not been reported in the literature in individuals with VCL-related conditions. ClinVar contains an entry for this variant (Variation ID: 179157). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The glycine amino acid residue is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant is not present in population databases (ExAC no frequency). This sequence change replaces alanine with glycine at codon 446 of the VCL protein (p.Ala446Gly). The alanine residue is weakly conserved and there is a small physicochemical difference between alanine and glycine. -
not specified Benign:1
Likely benign, criteria provided, single submitterclinical testingLaboratory for Molecular Medicine, Mass General Brigham Personalized MedicineAug 07, 2013Ala446Gly in exon 10 of VCL: This variant is not expected to have clinical signi ficance due to a lack of evolutionary conservation. Of note, 4 mammals (rat, mou se, cow, and hedgehog) have a glycine (Gly) at this position despite high nearby amino acid conservation, suggesting that this change is tolerated. -
