A variant of uncertain significance has been identified in the RAF1 gene. The I122T variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. This variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The I122T variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Additionally, this substitution occurs at a position where amino acids with similar properties to Isoleucine are tolerated across species. Nevertheless, in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Furthermore, I122T has been observed in one other individual referred for Noonan syndrome genetic testing at GeneDx who also harbored a pathogenic variant in the PTPN11 gene, although no segregation data is available to further clarify the role of the I122T variant in disease.Therefore, based on the currently available information, it is unclear whether this variant is pathogenic or rare benign. -

The p.I122T variant (also known as c.365T>C), located in coding exon 3 of the RAF1 gene, results from a T to C substitution at nucleotide position 365. The isoleucine at codon 122 is replaced by threonine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
