Ataxia-telangiectasia syndrome Uncertain:2
- -
This sequence change replaces valine, which is neutral and non-polar, with isoleucine, which is neutral and non-polar, at codon 2873 of the ATM protein (p.Val2873Ile). This variant is present in population databases (rs730881327, gnomAD 0.002%). This variant has not been reported in the literature in individuals affected with ATM-related conditions. ClinVar contains an entry for this variant (Variation ID: 181900). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

This variant is denoted ATM c.8617G>A at the cDNA level, p.Val2873Ile (V2873I) at the protein level, and results in the change of a Valine to an Isoleucine (GTA>ATA). This variant has been observed in at least one individual with a personal and/or family history suggestive of a hereditary cancer syndrome undergoing genetic testing via a multi-gene panel and in an individual with melanoma (Mu 2016, Yehia 2018). ATM Val2873Ile was not observed at a significant allele frequency in large population cohorts (Lek 2016). This variant is located in the catalytic region of the Kinase domain (Lavin 2004, Stracker 2013). In silico analysis, which includes protein predictors and evolutionary conservation, supports that this variant does not alter protein structure/function. Based on currently available evidence, it is unclear whether ATM Val2873Ile is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -
Hereditary cancer-predisposing syndrome Uncertain:2
This missense variant replaces valine with isoleucine at codon 2873 of the ATM protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has been identified in 2/250348 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.V2873I variant (also known as c.8617G>A), located in coding exon 58 of the ATM gene, results from a G to A substitution at nucleotide position 8617. The valine at codon 2873 is replaced by isoleucine, an amino acid with highly similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
ATM-related disorder Uncertain:1
The ATM c.8617G>A variant is predicted to result in the amino acid substitution p.Val2873Ile. This variant was reported in an individual with melanoma (Table S2, Dalmasso et al 2021. PubMed ID: 34262154). This variant is reported in 0.0018% of alleles in individuals of European (Non-Finnish) descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
