This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 975 of the ATM protein (p.Asn975Ser). This variant is present in population databases (rs730881354, gnomAD 0.006%). This missense change has been observed in individual(s) with breast cancer and ovarian cancer (PMID: 19781682, 34326862, 36029002). ClinVar contains an entry for this variant (Variation ID: 181937). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Not Available"; PolyPhen-2: "Benign"; Align-GVGD: "Not Available". The serine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The p.N975S variant (also known as c.2924A>G), located in coding exon 19 of the ATM gene, results from an A to G substitution at nucleotide position 2924. The asparagine at codon 975 is replaced by serine, an amino acid with highly similar properties. This alteration has been reported in at least one subject in a study of 13087 breast cancer cases and 5488 control individuals in the UK (Decker B et al. J Med Genet, 2017 11;54:732-741). This alteration has also been detected in 1/2531 breast cancer cases and 0/2245 controls (Tavtigian SV et al. Am J Hum Genet, 2009 Oct;85:427-46). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
This missense variant replaces asparagine with serine at codon 975 of the ATM protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with breast cancer (PMID: 19781682). This variant has been identified in 2/250842 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2924A>G has been reported in an individual affected with Breast Cancer without evidence for causality (Tavtigian_2009). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Seven clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
