Variant summary: SMAD4 c.917A>G (p.Asn306Ser) results in a conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 2e-05 in 251384 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.917A>G has been reported in the literature as a VUS in an individual with an appendiceal tumor, family history of BRCA1/BRCA2-negative breast cancer, who also carried a homozygous pathogenic CHEK2 variant (Kidambi_2017). This report does not provide unequivocal conclusions about association of the variant with Juvenile Polyposis Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. ClinVar contains an entry for this variant (Variation ID: 182870). Based on the evidence outlined above, the variant was classified as uncertain significance. -

This missense variant replaces asparagine with serine at codon 306 of the SMAD4 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been identified in an individual affected with colorectal cancer (PMID: 26900293), as well as an individual affected with appendiceal canced, who has a family history of breast cancer and also carried a pathogenic CHEK2 variant (PMID: 28283864). This variant has been identified in 5/251384 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome;C4707243:Familial thoracic aortic aneurysm and aortic dissection Uncertain:1
The p.N306S variant (also known as c.917A>G), located in coding exon 7 of the SMAD4 gene, results from an A to G substitution at nucleotide position 917. The asparagine at codon 306 is replaced by serine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Identified in an individual with a personal history of appendiceal tumor and family history of breast cancer who was also homozygous for a pathogenic CHEK2 variant (Kidambi et al., 2017); This variant is associated with the following publications: (PMID: 26900293, 10636916, 28283864) -

This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 306 of the SMAD4 protein (p.Asn306Ser). This variant is present in population databases (rs730881953, gnomAD 0.007%). This variant has not been reported in the literature in individuals affected with SMAD4-related conditions. ClinVar contains an entry for this variant (Variation ID: 182870). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SMAD4 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
