The variant received 17 classification points (ACMG Germline Pathogenicity v2019): 17P and 0B. PS3PM1PM2PM5PP2PP3_ModeratePP5_Strong

The NM_172107.4(KCNQ2):c.740C>T (p.Ser247Leu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant is absent from the gnomAD population database at sites with sufficient sequencing coverage. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. Variant has been reported in ClinVar as Pathogenic/Likely Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV003443824: Experimental studies have shown that this missense change affects KCNQ2 function (PMID:32770121).". A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.S247P: Likely_pathogenic (ClinVar VariationId 813771, 1 star) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.S247*: not_provided (ClinVar VariationId 21799); p.S247= (synonymous): Likely_benign (ClinVar VariationId 339335, 1 star) This exact variant is curated in the UniProt human variants database as Uncertain Significance; it is also listed as a COSMIC curated somatic variant.

Genes affected
KCNQ2 (HGNC:6296): (potassium voltage-gated channel subfamily Q member 2) The M channel is a slowly activating and deactivating potassium channel that plays a critical role in the regulation of neuronal excitability. The M channel is formed by the association of the protein encoded by this gene and a related protein encoded by the KCNQ3 gene, both integral membrane proteins. M channel currents are inhibited by M1 muscarinic acetylcholine receptors and activated by retigabine, a novel anti-convulsant drug. Defects in this gene are a cause of benign familial neonatal convulsions type 1 (BFNC), also known as epilepsy, benign neonatal type 1 (EBN1). At least five transcript variants encoding five different isoforms have been found for this gene. [provided by RefSeq, Jul 2008]

Classification according to ACMG Germline Pathogenicity v2019

Classification was made for transcript

Our verdict: Pathogenic. The variant received 17 points.

PS3
Well-established functional study supports damaging effect (PS3); SCV003443824: Experimental studies have shown that this missense change affects KCNQ2 function (PMID: 32770121).
PM1
Missense neighbourhood hotspot (≥2 P/LP within ±8 AA, OR ≥ 5 vs. background) — PM1; In-domain: 0 pathogenic, 0 benign rare missense variants.; ±8 AA neighbourhood: 27 pathogenic, 0 benign (OR vs. background: 275.0).
PM2
