Genes affected
KHDC3L (HGNC:33699): (KH domain containing 3 like, subcortical maternal complex member) The protein encoded by this gene belongs to the KHDC1 family, members of which contain an atypical KH domain that may not bind RNA like canonical KH domains. This gene is specifically expressed in the oocytes, and recent studies suggest that it may function as a regulator of genomic imprinting in the oocyte. Mutations in this gene are associated with recurrent biparental complete hydatidiform mole. [provided by RefSeq, Dec 2011]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most 50 bp of the penultimate exon, not predicted to undergo nonsense mediated mRNA decay. Fraction of 0.489 CDS is truncated, and there are 1 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 6-73363259-C-T is Pathogenic according to our data. Variant chr6-73363259-C-T is described in ClinVar as [Likely_pathogenic]. Clinvar id is 208592.Status of the report is criteria_provided_single_submitter, 1 stars. Variant chr6-73363259-C-T is described in Lovd as [Likely_pathogenic].

Submissions by phenotype

Hydatidiform mole, recurrent, 2 Pathogenic:1
Likely pathogenic, criteria provided, single submitterclinical testingLaboratory for Molecular Medicine, Mass General Brigham Personalized MedicineOct 16, 2014The p.Gln112X variant in KHDC3L has not been previously reported in individuals with disease or in large population studies. This nonsense variant leads to a premature termination codon at position 112, which is predicted to remove roughly half of the protein and therefore likely to result in a null or loss of function effect. Biallelic null variants (variants in both copies of the gene) have been shown to cause hydatidiform mole (Parry 2011, Reddy 2013). In summary, although additional studies are required to confirm a null effect, the p.Gln112X variant in KHDC3L is likely pathogenic for hydatidiform mole in an autosomal recessive manner. -
