Variant summary: HCN4 c.724C>T (p.Arg242Cys) results in a non-conservative amino acid change located in the Ion transport N-terminal domain of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 3.2e-05 in 251140 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.724C>T has been reported in the literature in a family affected with hypertrophic cardiomyopathy, however authors classified the variant as VUS (example: Robyns_2017) . These report(s) do not provide unequivocal conclusions about association of the variant with Sick Sinus Syndrome 2. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 29255176). ClinVar contains an entry for this variant (Variation ID: 240173). Based on the evidence outlined above, the variant was classified as uncertain significance.

This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 242 of the HCN4 protein (p.Arg242Cys). This variant is present in population databases (rs745880598, gnomAD 0.02%). This variant has not been reported in the literature in individuals affected with HCN4-related conditions. ClinVar contains an entry for this variant (Variation ID: 240173). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt HCN4 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The p.R242C variant (also known as c.724C>T), located in coding exon 1 of the HCN4 gene, results from a C to T substitution at nucleotide position 724. The arginine at codon 242 is replaced by cysteine, an amino acid with highly dissimilar properties. This alteration has been reported in a primary arrhythmia and cardiomyopathy cohort (Robyns T et al. Eur J Hum Genet, 2017 12;25:1313-1323). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
